ALIGN: Supportive Care
The ALIGN Supportive Care program was created to improve the care and treatment of patients with cancer. Our program works to help address your concerns, understand your goals of care, and treat your symptoms from a holistic physical, spiritual, and emotional perspective


Supportive Care: Outpatient Clinic
In our outpatient clinic, we will work to identify your goals of care, treat your symptoms, and work with your oncology team.

Pain Assessment - where we review your pain and determine the best approach to management

Symptom Assessment - where we understand the impact of other symptoms such as nausea, vomiting, fatigue, and sleeping issues while developing a treatment plan.

Spiritual Assessment - where we seek to understand your spiritual needs and concerns

Advance Care Planning - where we work with you to complete documents, letting us know your wishes

Comprehensive Plan and Follow Up - based on your evaluation, we will create a comprehensive plan to treat your health care goals and address any issues we found during our exam.

The AMEN (American Medical Evangelical Network) program partners with faith-based organizations to help address your spiritual needs.


Supportive Care: Inpatient Care
When your admitted to the hospital for supportive care, we will work to get control of your symptoms and understand more about your goals of care.

Goals of Care - where we determine your goals of care ​ and how we can help you achieve them

Comprehensive Pain Assessment - in the hospital setting, we can work to get your cancer-related pain under control and identify ways to improve control once out of the hospital

Symptom Assessment - Some cancer symptoms, such as nausea and vomiting, may require an inpatient stay. We will provide a comprehensive assessment and discuss treatment options to improve your care.

Spiritual Assessment - Chaplains will be available to help you with your spiritual needs and concerns throughout your treatment

Advance Care Planning - we will review and complete documents that let us know your wishes and how we can meet your needs

Comprehensive Plan and Follow Up - we will work to help you get set up with our outpatient clinic so you will have a place for continued care and support after you leave the hospital
