SAFE Start - Initial Visit
Our Standardized Age-Friendly Evaluation (SAFE) Start initial visit will provide you with a comprehensive overview of your health, age-friendly guidance, and a plan to meet your goals of care.
Fall Prevention Clinic
Falls can lead to broken bones, loss of mobility, and other complications for older adults. Our clinic works to identify your fall risk and help you gain the strength you need to be functional and independent.
Memory Care Clinic
Screening for memory issues is vital as subtle signs can often go unnoticed. Our clinic will work with you to understand the results of your testing and things we can do to improve your memory.
Delirium Care Clinic
Getting delirium while in the hospital is associated with worse outcomes for older adults, including an increased risk of death after discharge. Our delirium clinic works with patients who have developed delirium to identify risk factors that may have caused it and to optimize their health to reduce these outcomes.
Frailty Care Clinic
Frailty is a complex syndrome in which the body becomes weaker and more vulnerable to stress and disease. We work to identify frailty in older adults and create a comprehensive plan to strengthen and rebuild their health.

Delirium Clinic
Delirium is a serious and often underrecognized geriatric syndrome that can increase the risk for memory issues and death after hospitalization
Visit Goals
The Delirium Clinic is a new model of care for older adults after hospitalization. Our goal is to perform a comprehensive geriatric assessment and identify risk factors that may have contributed to your development of delirium. Our hope is that by acting early and thoroughly, we will improve your health and recovery.

Frailty Clinic
As we age, some people develop frailty, in which their bodies become weak and more susceptible to infection and disease. By working early to identify risk factors, we can create a plan to improve frailty and decrease our risks.
