GO-FIT: Age-Friendly Surgical Care

The GO-FIT program is designed to identify and improve risk factors for surgery in older adults. By taking a comprehensive approach to care before and after surgery, we hope to reduce complications, improve outcomes, and enhance recovery. 

Comprehensive Age-Friendly Assessment 

During your initial visit to the GO-FIT clinic, we will perform a comprehensive assessment to identify and review risk factors and vulnerabilities for your upcoming surgery 

Frailty Assessment 

Fraility can increase your risk for poor outcomes following surgery. We will work to identify if you have or are at risk of frailty and develop a treatment plan to strengthen you for surgery. 

Delirium Risk Assessment 

Delirium is a serious complication that can occur after surgery. We will work to identify risk factors and develop a treatment plan that helps to reduce your chances of delirium.  

Memory Assessment 

Understanding your baseline memory is essential before surgery. During your initial assessment, we will perform a comprehensive memory assessment and develop a treatment plan based on those findings. 

Goals of Care and Advance Care Planning 

We will work to understand more about your goals of care with this surgery and help you plan for the unexpected by filling out advance care planning documents that let us and your family know what your treatment preferences are.

GO-FIT works to coordinate your care across the healthcare team to make sure you get the high-quality, patient-first care you deserve 

Inpatient Age-Friendly Assessment 

After your surgery, our team will follow up with you in the hospital to identify and continue the treatment plan we developed for you. 

Functional and Mobility Assessment 

We will work with physical therapy, the surgical team, and other individuals to ensure your mobility and functional needs are met after surgery. By identifying needs early, engaging in proactive mobility, and working towards a discharge plan, we will help improve your functional status and recovery. 

Delirium Precuations (iDREAM)

Based on your risk factors, you will be placed on a proactive delirium prevention protocol to help reduce the risk of developing this condition. 

Goals of Care and Advance Care Planning 

We will make sure that your goals of care and plans we have developed are respected by the treatment teams during and after your hospital stay. 

Dischage Planning 

We will work with you to ensure that you are seen in our geriatric clinics as needed to help you during your recovery process. We will strive to make sure you are not "lost" in the system and that you get the care you deserve.
