iDREAM: Delirium Care 

The iDREAM program was created to identify and reduce the risk of delirium in older adults. By applying systems-based engineering and using the latest evidence, our program works to proactively improve patient care. 

Delirium Risk Screening 

Our team works to identify patients who may be at risk for delirium and starts proactive delirium precautions on admission to the hospital. 

Delirium Assessments

Delirium can occur at any point during a hospital visit. For the duration of their stay, all patients over the age of 65 are screened twice daily.  This level of monitoring allows for the identification of early any changes suggestive of delirium.  

Nurse-Driven Delirium Interventions 

Multiple interventions done early in delirium can improve outcomes and reduce the duration of the illness. Once delirium has been identified through our screening, nurses will start a comprehensive treatment plan. The plan is designed based on the latest evidence to improve outcomes for older adults with delirium. 

Geriatric Consults  

All patients who develop delirium will be seen by a Geriatrician. These doctors and nurse practitioners have special training in the care of older adults. They will work with your treatment team to help you recover from delirium and identify other risk factors that can be treated to enhance your care. 

Outpatient Delirium Clinic 

Because delirium is assoicated with increased risk of memory issues and mortality after leaving the hosptial, patients will be able to be seen in our geriatric clinic to review your risk factors and create a treatment plan to ensure a smooth recovery.

SAFE Start: Age-Friendly Assessment 

After your hospital stay, you will have the option to meet with our outpatient team to have a comprehensive age-friendly assessment. During this assessment, we will work to identify risk factors and treatment options that lead to your delirium. 

Functional and Mobility Assessment 

Being in the hospital can reduce mobility and independence. Our team will test for frailty and fall risks to ensure your safety at home during your recovery. 

Memory Assessment 

Memory issues can develop after having delirium. During your visit, we will perform a baseline test and set up a treatment plan to help monitor for any changes over time. 

Goals of Care and Advance Care Planning 

We will make sure that your goals of care and plans we have developed are respected by the treatment teams during and after your hospital stay.
